Please submit the following:

The form HERE, appropriately modified for what you are submitting. This will be cut and pasted into the source that generates the program. Just send this to me in an e-mail.
A video that is a three-minute advertisement for your talk. See for details on how to submit. Notice you fill the YouTube ID into the form.
A PDF for your "In Conclusion" slide. Please e-mail me this PDF and fill the name of the file in the form.
A PNG for your "In Conclusion" slide. Please e-mail me this PNG and fill the name of the file in the form.
Your primary resource, which could, for example, be a longer video, a related paper, or a set of graphs with some explanation. See for details on how to submit the longer video. Please fill the information regarding this resource in the form.
