| Late Add Survey | 8th day of class or instructor discretion | This survey will be available 8:00AM CST on the 5th class day. Students will need to fill one survey per course that they are trying to late add. Only for C S courses, both C S majors and non-C S majors/Elements students. |
| C S Honors Course Request | 4th Class Day | Students will need to fill out a survey for EACH course that they are requesting. This survey will be active on December 16th (for the Spring 2025 semester) to allow all grades for the Fall 2024 semester to be posted, as instructors need to see grades from the previous semester. Incoming new students will not need to submit an academic summary. C S majors only. |

Research Forms
Prior to registration for C S 370, the C S 370 contract will need to be turned via the online link. The student will be added to the course upon approval of the faculty advisor. Please be aware this form needs to have emailed permission by the supervising instructor attached via PDF or screenshot OR have supervising faculty instructor sign the contract.
NOTE: C S 370/370F can only be taken once for a letter grade/credit. You can continue research and register for C S 370/370F again, however it will be on a pass/fail basis.
Contract must be completed prior to registering for C S 370. These are due by the 4th class day of fall or spring semesters and by the 2nd class day of summer semesters.
C S Honors Thesis (C S 379H)
Contract must be completed and turned in via the online link prior to registering for C S 379H. These are due by the 8th class day of fall or spring semesters and by the 2nd class day of summer semesters.
The C S 379H request will be made through DocuSign. This will automate the process of routing the form to the appropriate faculty member for approval. Please be sure to read the instructions on the first page of the online form.
